Employee Health Screenings

In collaboration with SISC and Quest Diagnostics, FSD employees enrolled in a SISC medical plan will be eligible to participate in the Biometric Screenings and receive a $25 Amazon Gift Card. Appointments are required to earn a gift card. Please click here to schedule a biometric health screening appointment.

In order to participate in the health screenings and receive a $25 Amazon gift card you must:

• • Be enrolled in a FSD SISC medical plan. 
  • Must be active employee or dependent of an active enrolled employee. Retirees are not eligible to participate

If you are not able to attend an onsite wellness screening at FSD on April 10th or April 24th, you can make an appointment at a Quest Patient Service Center through the end of June.
