Eligible employees may enroll in a Delta Dental PPO or a DeltaCare USA HMO dental plan. If you choose the DeltaCare USA HMO plan, you must designate a dental office/name and dental office ID code.

For Dental coverage, there are no member ID cards. The provider verifies your eligibility through your SSN. You may register online through www.deltadentalca.org and print a temporary member ID card if you would like to use that (the dental office will still require your SSN).
